Self-Management Support: Patient Engagement

Self-Management Support: Patient Engagement is a live, all-day course designed to close a well-documented professional practice gap among care team members working in care management, care coordination, and Patient-Centered Medical Home settings. Grounded in evidence that fewer than 12% of U.S. adults have proficient health literacy skills and that only 7% of trained providers demonstrate intermediate or advanced motivational interviewing competence, this course equips participants with the knowledge, skills, and practice tools needed to meaningfully engage patients in their own care.

Participants engage in a progressive sequence of active learning experiences, including
role-playing exercises, a simulated interview with a trained, standardized patient, and receive immediate, individualized feedback on applied skills.

Upon successful completion of this course, participants will be able to:

  • Apply motivational interviewing (MI) techniques to effectively engage and empower patients and their support systems to promote health behavior change
  • Develop awareness and understanding of health literacy and the implications of cultural and ethical considerations on patient goals and preferences
  • Create a self-management action plan utilizing shared decision-making principles
  • Implement evidence-based interventions and advocate for the right care, at the right time, and in the right place
